| Anotation | The project focuses on the use of follow-up social services by clients with mental illness who have completed hospitalization in the Psychiatric Hospital in Opava. After the end of hospitalization in the psychiatric ward, not every patient can return to his/her home environment, or he/she cannot do without further help of follow-up social services. However, the transition from medical psychiatric care to follow-up social services is currently not seamless (Páv et al., 2017), many clients return to the hospital psychiatric ward after a short or longer time, or do not access the necessary social services after discharge from the hospital at all. The aim of this project is therefore to find out how follow-up social services are used after discharge from a psychiatric hospital, what are the main obstacles and shortcomings in the current process of establishing social services after psychiatric hospitalisation, and what are the main reasons for rehospitalisation of clients with mental illness from follow-up social services back to a psychiatric hospital. The theoretical framework of the project is based on the main principles of the Reform of Psychiatric Care (Winkler et al., 2013), which aims to improve the overall quality of life of patients with mental illness, with an emphasis on greater inclusion of people with mental illness in community care, early intervention, strengthening of community mental health care programs including outreach teams and support for people with mental illness. This care is to be multidisciplinary, systematic and coordinated, consisting of a team of health and social workers who will work together to support the person's recovery and integration back into normal life (Lidinska, P., 2017). In order to achieve the research aim, a primarily quantitative research strategy will be used, complemented by qualitative interviews with experts of related social services. |
